Training Verification

Physicians who trained in Internal Medicine and/or its specialties and subspecialties at the University of Toronto Department of Medicine may request verification of training taken in the Department. Verification services can include confirmation letters and/or forms, evaluations forms and letters, etc. There is an administrative fee for this service. (Please note, the Department cannot verify mixed and/or rotational internships. For assistance with verification of internships, please contact the relevant hospital medical education office or the College of Physicians & Surgeons of Ontario.)

Requests for verifications must be accompanied by the appropriate fee and the requesting physician’s signed authorization to release the information. A schedule of fees is provided below.

Training can be verified for physicians who trained in the Department of Medicine’s programs listed below.

Payment may be made by money order or certified cheque in Canadian or American funds and must accompany the request for verification. Certified cheque or money order should be made payable to the University of Toronto, Department of Medicine. Personal cheques and cash will not be accepted. Mail requests along with payment to

Fee Schedule

Fees are applied according to the year a physician was last enrolled in the Department as follows:

For physicians who registered as trainees in the Department of Medicine within the last 5 years the following fees apply –

Verification of training years only (form)	no charge
Forms requiring assessment of training and/or evaluating standing (including absence, probation, etc.)	$ 50 CAD or USD
Letters confirming training years only	$ 50 CAD or USD
Letters confirming and assessing training and/or evaluating standing (including absence, probation, etc.)	$100 CAD or USD
By Priority Courier - add CAD $20 for delivery in Canada - add CAD or USD $40 for international delivery Notarization by Notary Public - not provided Notarization by Commissioner of Oaths - no charge

For Physicians who were not registered as trainees with the Department of Medicine within the last 5 years the following fees apply –

Verification of training years only (form)	$ 50 CAD or USD
Forms requiring assessment of training and/or evaluating standing (including absence, probation, etc.)	$100 CAD or USD
Letters confirming training years only	$100 CAD or USD
Letters confirming and assessing training and/or evaluating standing (including absence, probation, etc.)	$200 CAD or USD
