Procedures Teaching in CRISP

Design

The Procedures module is geared to assist the development of competence, at the earliest stages, in bedside procedures.  Using a low-tech simulation setting, PGY-1 trainees learn the step-by-step mechanics of lumbar puncture, thoracentesis, central line insertion, and bone marrow aspiration/biopsy.

The module occurs in two parts.  The first part is a four hour session in which residents are taught each step of the procedure by clinical expert tutors, in small groups (5-7 trainees at a time).  They then are given time to practice each procedure, under the direct supervision of their tutor, with attention paid to the pure mechanics involved, ie. the correct locations, angles, forces and sequence of steps to be used.  One-to-one feedback is given with the aim of correcting any flaws in technique.

The second part is another session which reviews the techniques learned in the first part, and then assesses the trainees' performance in a formative evaluation.  Trainees are given guidance as to how to improve further as they practice the procedures in clinical settings, under supervision, after the CRISP session is over.

The teaching emphasis is on the interactions between the tutors and trainees, and on trainee reflection between sessions and afterward, with the goal of the accelerated development of competence in these four procedures.  Reflection and practice are actively encouraged between sessions and afterward.
